A three-year-old child has died following an alleged medical error at AIIMS Bhopal, prompting a police investigation and raising concerns about patient safety protocols at the hospital. Following an investigation, authorities say a case was filed after a substance that was supposed to be used in a laboratory was given through the child’s IV line during treatment.
The child from Bina had been taken to the hospital in 2025 for medical treatment, officials said. When the child was being treated, a formalin solution that is normally used by doctors to preserve bone marrow for analysis was reportedly fed into the inside of the child’s intravenous line rather than the lab.
Later tests and investigation reports indicated the child’s blood level had been taken up, and that his health worsened as a result. The child went on to die despite treatment.
After finding, the police of Bagh Sewania have lodged a case under Section on Negligence causing Death. Authorities have said the incident is being investigated and that more evidence will be gathered from the investigation for a determination of responsibility.
Details are also being referred to the appropriate medical regulatory authorities for investigation, authorities have said. Disciplinary and administrative measures against others responsible would be determined through proper procedures after an opportunity for due process is afforded.
The incident has sparked discussion about hospital safety procedures, medication handling practices, and the importance of strict verification protocols in healthcare settings. Authorities and investigators are likely to be able to investigate the circumstances surrounding the incident to see how the error was made and if there were any errors in procedure that led to the tragedy.
